Abstract
This research was successfully carried out with the participation of two manufacturing companies in Malaysia; Konway Printing Company Sdn. Bhd. and Leader Electrical Appliances Manufacturing Bhd. The goal was to introduce and facilitate the integration of Failure Mode and Effect Analysis (FMEA) and Statistical Process Control (SPC) for improving product quality and reliability. A mechanism for integrating FMEA and SPC was developed. The case study method enabled the mechanism to be tested in actual manufacturing environments. Actual barriers encountered by the participating companies during their course of integration of FMEA and SPC were unearthed. This method required the researcher to spend one and a half years in these companies. Solutions to the barriers generated and tested for practicability. FMEA and SPC were applied to actual quality problems. The post implementation results showed an improvement in the levels of quality problems. A FMEA software which provides guidance on FMEA implemention process as well as document the results for the implementation process was also developed. Companies can use the software to train employees on the usage of FMEA and SPC. The software can be downloaded at http://www.engusm.allhere.com at no cost; thus offering an alternative to the use of commercially available FMEA software.
