dc.description.abstractFailure Mode and Effect Analysis (FMEA) is one of the effective tools widely employed for failure identification and prevention. Risk ranking in FMEA is an ambiguous task. This is due to the information used for ranking risk are either subjective or difficult to obtain in nature. To overcome this problem, fuzzy logic was technique incorporated in FMEA risk ranking by Bolwes and Pelaez (1995). In this ' thesis, several improvements and new suggestions have ~een made to further improve the method suggested by Bolwes and Pelaez. To test the effectiveness of these new methods, case studies were carried out in an actual plant environment. There are 3 case studies were carried out namely the TNGB power plant condenser system, Motorola solder past~ printing and using the Linguistic Ordered Weighted Averaging (LOW A) method. Overall this thesis has succeed in improving the risk ranking method in FMEA, enhancing the decision making method regarding the ranking values, introducing a I method to represent uncertainty in ranking, providing more flexibility of the ranking method and guiding to outline the preferred method to be utilised according to situation.en_US
