Abstract
A parallel design randomized clinical trial was conducted at Universiti Sains Malaysia (USM) to evaluate the dento-alveolar and skeletal changes brought about by a three month night-time retention period after twin block therapy and before fixed appliances are bonded. 800 children at five primary schools in Kota Bharu and 32 patients from USM orthodontic clinics were screened for eligibility to participate in the trial. Inclusion criteria required children to be of Malay ethnicity, 10 to 15 years of age, have an overjet of 5mm or greater and a Class II malocclusion on a skeletal Class II base. Exclusion criteria were any history of orthodontic treatment, craniofacial deformities or craniofacial surgery. 43 children met the criteria and were invited to participate in the trial. 34 consented, of which 8 dropped out during the twin block stage of treatment. The remaining 26 completed twin block therapy and were randomized using block randomization, 13 per group, into two groups. Children in group A had fixed appliances bonded immediately whereas those in group B continued wearing the twin block at night for three months, after which fixed appliances were bonded. Lateral cephalometric radiographs were taken before twin block therapy (T1), after therapy was completed, before randomization (T2) and six months after bond-up of fixed appliances (T3). Pancherz and Steiner‟s analyses variables were used for analysis of the radiographs. Paired t test showed several statistically significant dento-alveolar and skeletal changes took place during time period T2 until T3 for children in group A. In contrast, only condylar head position for group B children exhibited statistically significant change.
