Abstract
The estate health care system is the provider of health services to the estate population in
our country. It is not under the jurisdiction of the Ministry of Health Malaysia and is man-
aged privately by the respective estate management. There is not much information that is ---
: known about this health care system. This is the reason why this study on the healthcare
management system of the estate setting .i n Peninsula Malaysia is. conducted. i"~'
This study involved 48 estate medical assistants from 48 estates in Peninsula Malaysia. The
study was conducted using the mail survey method. The response rate for this survey was
52.1% or 25 respondents. The respondents were queried on various aspects that covered
the management of the estate healthcare setting. Among the aspects queried are demography
information about the estate hospital/clinic and medical assistant, manpower information,
drugs and disease information, information about drug selection, procurement, receiving
and distribution and financial allocation information.
The data were compiled and analysed descriptively using Microsoft Excel and statistical
software, Statistical Package for Social Sciences (SPSS).
From the results, a few conclusions can be made. The estate medical assistants play the main
' role in the management and co-ordination of the health care delivery system provided by the
estate clinics and hospitals. However, the estate management makes the final decision on
important issues especially on issues pertaining to financial matters. Although these estate
medical assistants play such an important role, their training and knowledge on matters such
as the latest medical information and inventory management are greatly lacking. On the
prevelance of disease, the common diseases treated by the medical assistants are mainly diseases relating
to environment and work hazards and not chronic illness. The drugs used in these
settings also are mainly generic drugs and purchased individually. Although the health
services provided to the estate population is given free without any restrictions, the medical allocation provided for each individual is too low and insufficient.
lt is noted form this study that there are various shortcomings in the healthcare care system
in the estate settings in Peninsula Malaysia and more in-depth studies should be carried out
to improve this health system.
